Taiwan visa application form. Completed application form for Taiwan will be prepared within 5 business hours of order submission, and emailed to you to download, print and sign.

Original, signed Ukraine passport with one photocopy. Passport must have at least 6 months remaining validity on the date of travel and have at least 1 visa page clear of any markings.

Passport Photo: 2 Include a passport style photo, with a white background, taken within the last 6 months. You may also choose to upload a photo to your order for us to print. There is a surcharge associated with this service.

Original Proof of Status with one photocopy. Original Proof of Residency in the UK (stamped in passport or residence permit card) valid for a munimum of 3 months from departure date from Taiwan.

Itinerary. Copy of round trip tickets or confirmed itinerary.

Bank Statement. Copy of a recent bank statement showing proof of sufficient funds.

Invitation Letter. Copy of an invitation letter from family or friends in Taiwan and a copy of their passport and visa.

Employment Letter. Copy of a letter from your employer on business letterhead, with contact details, purpose and duration of the trip and that you will be returning to your current job. If you are self-employed, include a copy of your business license and tax return. If you are retired please submit proof of your retirement fund.
